This anonymous online survey will provide information for your team as to what areas of strength and weakness are currently being practiced by your team. Each member of an interdisciplinary team should complete this evaluation. Afterwards, the team leader will facilitate a meeting where the team member overall responses are shared. The team evaluation will serve as a baseline for the school year and will be re-administered at the end of the school year to measure team goal accomplishments.
