TEACHER EVALUATION PROCEDURES AND FORMS
Teacher Evaluation Training Materials (iTune U)
Each teacher in our building has been provided a link to iTunes U for accessing the e4E Teacher Evaluation training materials. You will receive one staff development point entered into your MyLearning Plan for reviewing the teacher evaluation materials. In order to access this information on your iPad you will need to download iTunes U by completing the following steps.
(Please note that if you already have iTunes U on your iPad skip steps 1 -3 and go to step 4.)
- Select the following link on your iPad for iTunes U
- You will be prompted to enter your iTunes account
- Download the application
- To request access to the teacher evaluation training materials click on the following link: e4E Teacher Evaluation iTunes Course
Forms
- Pre-Observation Goal Setting Form (Required: This form is used during the first pre-conference where date for observation is announced.)
- e4E Educator Observation Form (This form is used for formative classroom observations)
- Dodge City e4E Conference Form 9/13/13 (Not Required)
- Summative Evaluation Form (This form is used for scheduled summative evaluation which is submmited to HR)
